A 67-year-old female had a dual-chamber pacemaker implanted three years ago due to complete heart block. Her automatic mode switch is switched on at 170 bpm. During an episode of atrial fibrillation (with an atrial rate upwards of 300 bpm) her device fails to mode switch. Why might this be happening?
